Horizons Neurological Program is a specialty program for individuals with neurological impairments, focused on young adults ages 18–59 in Freehold Township. This state-based residential Medicaid program provides ongoing therapy services in physical therapy (PT), occupational therapy (OT), and speech (SPL) to some of the most vulnerable neuro populations.
Horizons treats a range of diagnoses including Traumatic Brain Injury (TBI), Stroke, MS, ALS, Parkinson’s, and Spinal Cord Injuries. These complex conditions require time to heal—our therapy team works to achieve meaningful gains, helping patients reach a new baseline and and then hopefully, exceeding it.
Admission requires meeting specific criteria. Once accepted, each patient receives an individualized care plan developed by their treating therapist, who determines the necessary disciplines and duration of care to support recovery.
